Filling out these forms ahead of time will help make your visits to Stanley Park Dental Care as quick and convenient as possible. If you have any questions when filling out the form our staff will happily assist you with any questions on the day of your appointment.
Financial information
All of our patients at Stanley Park Dental Care are entitled to a free consultation where treatment options, fees and general questions can be discussed. A dental treatment is an important decision so we want to make sure you understand exactly what procedures will be performed, the benefits, and the risks. Prior to proceeding with any treatment, fees and financial arrangements will be discussed with you and all your questions will be answered.
Stanley Park Dental Care accepts cash, debit, Visa, Master Card and Amex credit cards. All oustanding amounts are due at the time of service unless otherwise discussed.
We electronically file insurance claims. As an assignment office we submit your claims electronically to your insurance company and payment comes directly to us.*
* Dental claims will be submitted electronically and payment will be assigned to us under the circumstances that your insurance plan allows it. If you are unsure, please contact your insurance company or ask reception for assistance.
